  • FOR DISASTER OR EMERGENCY PURPOSES: I authorize the Onward 2 High School Summer Institute employees to provide medical services for my child in the event of an emergency. I understand and give permission for adult leaders to make a fair judgment and seek emergency action and emergency medical treatment in the event that they are not able to or do not have time to first contact a parent/emergency. I accept all liability and voluntarily release, discharge, and waive all claims against Onward 2 High School program, the Smet Foundation, its officers, trustees, directors, employees, and volunteers.*
  • MEDICATIONS

    The school shall not furnish medications, because of the risk of students sharing medications, students may not carry medications of any kind (prescription or non-prescription to be self-administered at school. A release stating the nature of medication, signed and dated by the doctor/parent must be provided. Medication administered at school must be in the original container and labeled. The day's dosage must be sealed in appropriate container. The student shall come to the designatd Onward 2 High School office for medication.


    The parent/guardian of a diabetic child must sign the Diabetic Consent Form and other appropriate medication permission forms and return them to the school.


    ALL MEDICATIONS MUST BE KEPT IN THE SCHOOL OFFICE. 

    No exceptions will be made to the procedure for medication. If parents/guardians do not provide the completed medication form with the prescribed medication, they will have to come to school and personally administer the medication.

  • Acknowledgement of Risks. I understand, acknowledge, and agree that there are certain risks inherent in Student’s participation in the Onward 2 High School Summer Institute, and that these risks include, but are not limited to: (1) injuries caused by slips and falls or walking on uneven surfaces; (2) damage to or loss of personal property; (3) exposure to or contracting infectious diseases, and any medical complications or secondary conditions resulting from the same; (4) the dangers inherent in any type of transportation, including walking, driving in private cars, riding vans or buses, or any other type of private or public transportation, such as pedestrian or vehicular accidents; (5) the dangers of being in a place open to the public; emotional distress and trauma; (6) minor injuries such as insect bites, scratches, bruises, and sprains; (7) risks associated with food consumption, including allergic reactions and foodborne illness; (8) injuries from exposure to the sun or the outdoors, such as sunburns, heat related illnesses, and insect bites; (9) catastrophic injuries including, permanent disability and death. The activities Student engages in during the Field Trip may be dangerous and include risks that are inherent and cannot be reasonably avoided without changing the nature of the activities. O2HS and its program leaders cannot foresee every possible contingency or completely eliminate all risk. 

  • Parental Permission for Access to the Internet

  • The Onward 2 High School Summer Institute is hosted at a Catholic high school site. A safe environment for all members of the community should be the hallmark of a Catholic School. This is accomplished, in part, by fostering a climate based on values that emphasizes the dignity of and respect for all persons. Words, actions, or depictions which violate the privacy, safety, or good name of others are inconsistent with the goal.

    This policy applies to communication, attachments or depictions through email, text messages or web site postings, where they occur on the school or program computer network or through private communications, which:

    Threaten, libel, slander, malign, disparage, harass or embarrass members of the school or program community;
    Are of a sexual nature; or in the partner high school employee's or O2HS employee's discretion, as applicable, cause harm to the school or their communities ("Inappropriate Electronic Conduct")

    Inappropriate Electronic Conduct shall be subject to the full range of disciplinary consequences. The site director shall investigate and act upon all reported or observed instances of inappropriate Electronic Conduct. Students, parents/guardians, members of the school or program staff and volunteers are expected to report promptly to the person in charge all suspended or observed instances of inappropriate Electronic Conduct. The school or Onward 2 High School program at any time may add additional rules and restrictions. The school and Onward 2 High School program have the right to monitor all use of electronic communications as set forth in the Archdiocesan Acceptable Use and the Responsibility Policy for Electronic Communications ("Archdiocesan AUP") and/or private school partners electrontic policies. For the Onward 2 High School Summer Insitute, all inappropriate conduct investigation shall be conducted by leadership of the program and possible school site. 

    I hereby release the Onward 2 High School Summer Institute sites; Cathedral High School, St. Mary's Academy, St. Anthony High School, St. Pius X - St. Matthias Academy and the Archdiocese of Los Angeles, its personnel and any other institutions with which it is affiliated, from any and all claims and damages of any nature arising from the unauthorized use of the system.

    I hereby give my permission for my child to use the Internet, subject to those policies and will not hold Summer Institute, partner schools listed above, or the Archdiocese of Los Angeles liable as a result of my child's use of the Internet on school premises. I understand that my child has agreed not to access inappropriate material in the internet.

  • Parent’s Authorization to Use Child’s Image, Name, Voice And/Or Work for Non-Commerical Purposes

  • Photos or Likeness of Students are Onward 2 High School Property. I authorize Onward 2 High School employees and its affiliates and/or to make video or audio recordings of the Student, or to use Student’s name, voice, photograph or likeness, in any and all Onward 2 High School publications or promotions in any media whatsoever, for any period of time that may extend beyond Student’s enrollment at Onward 2 High School, and without compensation from Onward 2 High School.

    In consideration for permitting the Student to participate in the field trip(s) with the Onward 2 High School (“O2HS”), an initiative of The John H. and Cynthia Lee Smet Foundation, on behalf of myself, Student, and our heirs, executors, administrators, and assigns, hereby agree to the following terms and conditions of this Field Trip Permission, Waiver, Release and Indemnity Agreement (“Agreement”).

    1. Voluntary Participation. I give permission for the Student to participate in the Field Trip(s). I understand that the Student’s participation in the Field Trip(s) is voluntary as part of Onward 2 High School’s voluntary Summer Institute program. 

    2. Transportation. I understand and agree that O2HS will arrange transportation to or from the Field Trip for Student via chartered bus, the program director’s personal vehicle, or another employee’s vehicle. 

    3. Acknowledgement of Risks. I understand, acknowledge, and agree that there are certain risks inherent in Student’s participation in the Field Trip(s), and that these risks include, but are not limited to: (1) injuries caused by slips and falls or walking on uneven surfaces; (2) damage to or loss of personal property; (3) exposure to or contracting infectious diseases, and any medical complications or secondary conditions resulting from the same; (4) the dangers inherent in any type of transportation, including walking, driving in private cars, riding vans or buses, or any other type of private or public transportation, such as pedestrian or vehicular accidents; (5) the dangers of being in a place open to the public; emotional distress and trauma; (6) minor injuries such as insect bites, scratches, bruises, and sprains; (7) risks associated with food consumption, including allergic reactions and foodborne illness; (8) injuries from exposure to the sun or the outdoors, such as sunburns, heat related illnesses, and insect bites; (9) catastrophic injuries including, permanent disability and death. The activities Student engages in during the Field Trip may be dangerous and include risks that are inherent and cannot be reasonably avoided without changing the nature of the activities. O2HS and its program leaders cannot foresee every possible contingency or completely eliminate all risk. I have had opportunities to discuss the Field Trip(s) with the O2HS.

    4. Assumption of Risks. I understand that these injuries or occurrences may arise from Student’s or other’s actions, inactions or negligence, conditions related to travel, condition of the Field Trip location(s), or the nature of the Field Trip(s). Nonetheless, I acknowledge and expressly assume all risks and dangers associated with Student’s participation in the Field Trip(s), whether described above, known or unknown, and inherent or otherwise. I take full responsibility for any injury, including the risks set forth in paragraph 3 above, which the Student may suffer, arising in whole or in part from Student’s participation in the Field Trip(s).

    5. No Exceptions to Group Travel Plans. Student will travel with the O2HS group according to the plans made by O2HS. I will not expect any exceptions to be made for Student because of personal or family plans.

    6. Student Conduct. I understand that the Student is expected to abide by all O2HS rules, policies, and procedures during the course of the Field Trip(s), and must follow all health and safety requirements and other instructions given to Student by any employee, volunteer, chaperone, representative, or agent of O2HS in charge of or supervising or chaperoning the Field Trip(s) (“Representative(s)”) while participating in the Field Trip(s). I understand and agree that Student’s failure to follow these rules, policies, requirements, and protocols may result in discipline up to and including expulsion from Summer Institute and/or termination of Student’s participation in the Summer Institute in the sole and absolute discretion of O2HS.

    7. Early Return of Student. If O2HS terminates Student’s participation in the Field Trip(s) for any reason, Student will immediately be sent home with a chaperone at my expense, or I may be required to travel to the destination of the Field Trip to pick up Student at my expense.

    8. Voluntary Release of All Claims. To the fullest extent permitted by law, I, on behalf of myself, the Student, and our heirs, executors, administrators and assigns, completely absolve, waive and release O2HS, its officers, trustees, directors, employees, volunteers, insurers, independent contractors, agents and representatives (collectively “the Released Parties”), from any and all claims, demands, damages, or liabilities, including, but not limited to, those for the ordinary negligence of a Released Party, claims for bodily injury, personal injury or illness, emotional distress, property damage, wrongful death, consequential damages, economic damages, and attorney’s fees and costs (“Claims”) which arise from or relate to Student’s participation in the Field Trip(s). This release, discharge, waiver and relinquishment also pertains to any instruction or supervision related to the Field Trip(s) on the part of OSP or Released Parties. However, this release does not apply to a Released Party to the extent a claim is caused by the willful misconduct or gross negligence of that Released Party.

    9. Release from Third-Party Liability.  I understand that O2HS is not an agent of, and has no responsibility for, any third party including any sponsor or entity that may provide any services, hospitality, public and/or private transportation, equipment, training, or activities associated with the Field Trip(s). I hereby relieve the Released Parties from any and all claims arising out of such third party liability, events, acts, or omissions.

    10. Indemnification/Hold Harmless. I acknowledge I may be held liable and responsible for any injury or death to another person or injury to property of another caused by me or the Student. To the fullest extent permitted by law, I hereby agree to indemnify and hold O2HS, the John H. and Cynthia Lee Smet Foundation, and any Released Party harmless from any and all Claims caused by the Student’s or my own negligent, intentional, or other conduct related to Student participating in the Field Trip(s). Such indemnification obligation shall not extend to a Released Party to the extent a claim is caused by the willful misconduct or gross negligence of that Released Party.

    11. Medical Care. Any adult accompanying Student on the Field Trip(s) may consent to the administration of medical or dental care to Student as deemed appropriate. I agree to assume full responsibility for the costs of such medical or dental care for the Student that may be required. I fully understand that this authorization is given in advance of any specific diagnosis, treatment or hospital care being required. I have read and fully completed the MEDICATION AUTHORIZATION AND RELEASE AND RELEASE FOR MEDICAL TREATMENT AGREEMENT provided by O2HS and acknowledge that the information contained therein is accurate and current. I also agree that any adult accompanying Student on the Field Trip(s) may administer medication as provided on the medical forms that I have filled out for Student.

    12. Integration, Severability, & Governing Law: This Agreement, in addition to the policies and rules provided at the Summer Institute program, and authorization for medication administration form, constitute and contain the entire agreement and final understanding between O2HS and me with regard to the subject matter of this Agreement. If any provision of this Agreement is held to be invalid or unenforceable by a court of competent jurisdiction for any reason, the remaining provisions shall continue to be valid and enforceable. The validity, interpretation, performance, and enforcement of this Agreement shall be governed by the laws of the State of California and Los Angeles County shall be the venue for resolution of all disputes.

    13. Modification: This Agreement cannot be modified unless approved in writing and signed by both OSP and the Parents.
